This agreement is for an Adult membership provided by Central Combat Sports Inc., located at 1565 Lauzon Road, Unit #2, Windsor, Ontario, N8S3N4, Canada.

All membership fees are non-negotiable and non-refundable. All fees are to be paid in full. Memberships require a six (6) month minimum commitment, specifically thirteen (13) bi-weekly payments at the agreed upon rate specified below. Any membership cancellation thereafter must be made in writing and will take effect after five (5) business days. Cancellation requests must be sent via email to [email protected].

By entering into this agreement, you are authorizing Central Combat Sports Inc. to begin collecting recurring membership payments, and any other costs arising from your membership, from any bank account or credit card provided. Should your payment source prove to be unreliable for any reason, you must immediately provide a viable replacement.

Recurring membership payments cannot be paused for any reason until the minimum commitment is fulfilled, except in the event that Central Combat Sports Inc. is unable to provide services for a period longer than 14 days. Any declined or insufficient payments will incur a penalty fee of $10.00 plus the balance owing. After one declined or insufficient payment, you or your child will be prohibited from attending classes until any outstanding balance is paid in full, and a reliable payment source is established.

Failure to pay the agreed upon membership fees may result in your file being sent to a collections agency. In the event of any dispute arising from a member’s failure to pay membership fees or any other money due to Central Combat Sports Inc., the prevailing party shall be entitled to recover all legal fees and costs incurred in connection therewith.

All members and guests must abide by any facility rules or policies, and follow any general or safety guidelines made available by Central Combat Sports Inc. Programs offered, class schedules, and hours of availability are subject to change without notice, and may change from time to time.

Visitors from out of town are not eligible for membership, including any advertised satisfaction guarantee. Private lessons or a short-term training pass must be purchased instead.

Central Combat Sports Inc. reserves the right to terminate the membership of any individual at its sole discretion. Central Combat Sports Inc. may terminate the membership of any individual that neglects the rules of Central Combat Sports Inc. or is involved in harmful behavior towards the members, guests or property of Central Combat Sports Inc.

Individuals will be responsible for all replacement costs for any theft of or damage caused to the equipment or property of Central Combat Sports Inc., whether intentionally, unintentionally or through negligent behaviour. Central Combat Sports Inc. may collect these costs from any payment source associated with the individual responsible at its sole discretion and without notice.

Release of Liability(Required)

I understand and agree.

There are potential risks for injury involved in the training and participation of any physical activity, especially martial arts. You must accept and fully assume all such risks, dangers and hazards, including but not limited to injury through physical activity and/or use of equipment and facilities, property damage or loss, the contraction of communicable disease, and even death, resulting from any participation in any services provided by or endorsed by Central Combat Sports Inc. You should discuss your or your child’s participation in this activity with a physician to determine any possible health implications.

In consideration to participate in the activities of Central Combat Sports Inc., you must agree as follows:

To accept and acknowledge that your or your child’s image may be recorded, reproduced and published through digital or print media for advertising purposes, without any entitlement to renumeration or compensation.

To waive any and all claim that you have or may have in the future against Central Combat Sports Inc., its directors, officers, employees, representatives, officials, executives, sub-contractors, guests or participants, all of whom are hereinafter collectively referred to as “releasees”.

To release the releasees from any and all liability for any loss, damage, disease, injury or expense that you may suffer or that your next of kin may suffer as a result of your or your child’s participation in any service provided by or associated with Central Combat Sports Inc. due to any cause whatsoever, including negligence, breach of contract or breach of any statutory or other duty of care. You acknowledge your responsibility to ensure adequate medical personal health, dental and accident insurance coverage, as well as protection of your personal possessions.

To hold harmless and indemnify the releasees from any and all liability for any damage to property of, or personal injury to, or costs, any third party, resulting from your participation in this activity.

This agreement shall be effective and binding upon your heirs, next of kin, executors, administrators, assigns or representatives in the event of your death or incapacity.

Total Including 13% HST

Payment will recur every 2 weeks.
