PRIVATE LESSONS

All fees are non-negotiable and non-refundable. All fees are to be paid in full. This agreement is for private instruction provided by Central Combat Sports Inc., located at 1565 Lauzon Road, Unit #2, Windsor, Ontario, N8S3N4, Canada.

A 24-hour written cancellation notice is required if you are unable to attend a scheduled session. Failure to provide sufficient notice or failure to attend a scheduled lesson will result in the lesson fee being applied in full.

Students who arrive late will only receive a lesson for the remainder of their allocated time slot. We advise students to arrive 5-15 minutes early.

Central Combat Sports Inc. reserves the right to immediately terminate the lesson of any individual or group involved in harmful behavior towards the instructors, members, guests or property of Central Combat Sports Inc. Any individual that causes damage to the equipment or property of Central Combat Sports Inc., whether intentionally, unintentionally or through negligent behaviour, will be responsible for all replacement costs for damaged items. Central Combat Sports Inc. may collect these costs from any bank account or credit card associated with the individual responsible for the damages at its sole discretion and without notice.

All students and guests must agree to abide by any facility rules or policies, and to follow any general or safety guidelines made available by Central Combat Sports Inc.

There are potential risks for injury involved in the training and participation of any physical activity, especially martial arts. You must accept and fully assume all such risks, dangers and hazards, including but not limited to injury through physical activity and/or use of equipment and facilities, property damage or loss, the contraction of communicable disease, and even death, resulting from any participation in any services provided by or endorsed by Central Combat Sports Inc. You should discuss your or your child’s participation in this activity with a physician to determine any possible health implications.

In consideration to participate in the activities of Central Combat Sports Inc., you must agree as follows:

To accept and acknowledge that your or your child’s image may be recorded, reproduced and published through digital or print media for advertising purposes, without any entitlement to renumeration or compensation.

To waive any and all claim that you have or may have in the future against Central Combat Sports Inc., its directors, officers, employees, representatives, officials, executives, sub-contractors, guests or participants, all of whom are hereinafter collectively referred to as “releasees”.

To release the releasees from any and all liability for any loss, damage, disease, injury or expense that you may suffer or that your next of kin may suffer as a result of your or your child’s participation in any service provided by or associated with Central Combat Sports Inc. due to any cause whatsoever, including negligence, breach of contract or breach of any statutory or other duty of care. You acknowledge your responsibility to ensure adequate medical personal health, dental and accident insurance coverage, as well as protection of your personal possessions.

To hold harmless and indemnify the releasees from any and all liability for any damage to property of, or personal injury to, or costs, any third party, resulting from your participation in this activity.

This agreement shall be effective and binding upon your heirs, next of kin, executors, administrators, assigns or representatives in the event of your death or incapacity.
