CENTRAL COMBAT SPORTS

Central Combat Sports Inc.'s annual education scholarship award for post-secondary students consists of a total financial award of up to $2000 CAD, available per calendar year. The total amount available per calendar year may be split among several applicants, and will be determined on a case-by-case basis. Applicants must be current members of Central Combat Sports Inc., in good standing, with at least 3 years of active membership to be eligible for a scholarship award. Students must be pursuing a post-secondary education program at an accredited college or university. Scholarship awards will be distributed to recipients in September of each calendar year.

Central Combat Sports Inc. reserves the right to modify, amend or cancel a scholarship award at any time without notice. Central Combat Sports Inc. will decide all award criteria, terms, and conditions at its sole discretion.

By applying for a scholarship, applicants agree that they will have no claim against Central Combat Sports Inc. on the basis of the failure to grant a scholarship award, the evaluation of applications, or the modification or revocation of a scholarship award.

Central Combat Sports Inc. may rescind a scholarship award if the recipient engages in conduct that is inconsistent with the values or policies of Central Combat Sports Inc., has been suspended or withdrawn from a school or course of study; has provided incorrect, false or misleading information or has withheld relevant information in the scholarship application or after receiving a scholarship award.

In consideration to participate in any activity provided by, endorsed, or associated with Central Combat Sports Inc., you must agree as follows:

To accept and acknowledge that your or your child’s image may be recorded, reproduced and published through digital or print media for advertising purposes, without any entitlement to renumeration or compensation.

To waive any and all claim that you have or may have in the future against Central Combat Sports Inc., its directors, officers, employees, representatives, officials, executives, sub-contractors, guests or participants, all of whom are hereinafter collectively referred to as “releasees”.

To release the releasees from any and all liability for any loss, damage, disease, injury or expense that you may suffer or that your next of kin may suffer as a result of your or your child’s participation in any service provided by, endorsed, or associated with Central Combat Sports Inc. due to any cause whatsoever, including negligence, breach of contract or breach of any statutory or other duty of care. You acknowledge your responsibility to ensure adequate medical personal health, dental and accident insurance coverage, as well as protection of your personal possessions.

To hold harmless and indemnify the releasees from any and all liability for any damage to property of, or personal injury to, or costs, any third party, resulting from your participation in this activity.

This agreement shall be effective and binding upon your heirs, next of kin, executors, administrators, assigns or representatives in the event of your death or incapacity.

You certify that all the information provided on the application form and in all the accompanying documents is true, accurate, and complete to the best of your knowledge. You understand that the provision of false information will render you ineligible for consideration for this scholarship award.

By signing and submitting this application, you attest that you are not relying upon any oral or written representations or statements other than what is set forth in this agreement.
